  • Improvement Project Reports from IHI.org users that describe improvement projects in their organizations.

 

We all learn from others' experiences testing and implementing changes in real settings — who should be on the team; what measures were tracked; which changes worked best or didn't work at all; and what lessons were learned.

Knowing the positive impact that continuity of care has on patient engagement, satisfaction and outcomes, Clinica Campesina uses several approaches that support continuity: consistent provider teams, open access scheduling, and group visits.

Cambridge Health Alliance: Where Children with Asthma Are Staying Out of the Hospital More
Through its participation in the Pursuing Perfection initiative, the Cambridge Health Alliance enhanced its already strong asthma program, creating a comprehensive Childhood Asthma Registry for its more than 1,500 young asthma patients, and implementing the Planned Care Model, which replaces the old reactive style of care with proactive asthma management.

Reducing Asthma Hospitalizations and Emergency Department Visits
By teaching residents and faculty the Chronic Care Model and applying this model of care in outpatient clinics, Maine Medical Center (Portland, Maine, USA) demonstrated the effectiveness of such a curriculum to improve patient care and reduce unnecessary health care costs by showing decreases in hospitalizations and Emergency Room visits for clinic patients with asthma.

Pursuing Perfection: Report from Cambridge Health Alliance on Improving Asthma Care
Although the seeds of Cambridge Health Alliance's asthma improvement work had already been planted, Pursuing Perfection provided an opportunity to take a more proactive approach to caring for children with asthma

AH! Improves Asthma Outcomes
By implementing a multidisciplinary coordinated team approach to asthma care, the AH! Asthma Health Program at The Barbara Bush Children’s Hospital at Maine Medical Center (Portland, Maine, USA) has decreased length of stay (LOS) for asthma patients on the Inpatient Unit (IPU) and decreased missed days of work and/or school for patients and their families.

Pursuing Perfection: School Nurses Key to Asthma Project at Cambridge Health Alliance
Cambridge Health Alliance (CHA), a health care system in Massachusetts (USA), has identified local school nurses as valuable members of their Pursuing Perfection Pediatric Asthma Project team.
