Raise Community Awareness Through Networking, Outreach, and Education
  1. Sponsor health fairs, screening, and education. Focus on special populations (ethnic, cultural, etc.).
  2. Share information with schools and daycare centers.
  3. Provide education to people at shelters and soup kitchens about asthma, exercise, etc.
  4. Work with faith-based organizations or other groups where your patients naturally gather.
  5. Recruit and train migrant/homeless community health workers (CHWs) to assist in education and screening outreach programs.
  6. Recruit and train CHWs to link patients and providers (e.g., chart flagging, referrals, outreach, education, and transportation).
  7. Have CHWs assist during support group meetings (e.g., providing testimonials or helping with peak flow demonstrations).

Tips
  • Have school-based staff participate in the asthma improvement team, or if the team is school-based, have primary care staff participate on team. Include athletic trainers or coaches on the team.
  • Some successful health fairs have included educational offerings for providers (first two hours are provider in-service with outside speakers, followed by a public event with outside speakers addressing patients).
  • Be sensitive to “turf” issues to maximize successful partnerships.
