Organization of Health Care:
Make Sure the Senior Leader Visibly Supports and Promotes the Effort to Improve Chronic Care
  1. Have the senior leader visit the clinical team.
  2. Have the senior leader speak about the improvement effort in all-staff meetings.
  3. Have the senior leader participate in writing the aims and goals of the asthma initiative and providing guidance for the clinical team.
  4. Have senior leader make monthly updates to the community.
  5. Present local morbidity/mortality (M/M) asthma data to Board of Directors to make a compelling case for the need to change current practice.
  6. Share results and community activity with Board of Directors quarterly.
  7. Use positive public relations opportunities to build support.
  8. Have physician champion share tested tools and interventions at provider meetings to engage providers’ interest and involvement.
  9. Have team members present progress reports at staff meetings.

Tips
  • Invite the senior leader to talk with other senior leaders.
  • Have senior leaders talk with spread teams when they are visiting.
  • Show the senior leader how the Model for Improvement can be used across the system.
  • Help the senior leader recognize how “good” this makes him/her and the center look in the eyes of the staff and the community.
  • Ask for Board of Directors’ support early and often.
  • Give feedback to Board regarding how support has made a difference.
  • Take advantage of Board members’ personal experience.
  • Spread the word through radio spots, newspaper articles, and community health fairs.
  • Ask senior leader/Medical Director to formally introduce the team to promote the effort, and urge other staff to help when asked.
  • As tools are tested and proven, share them with other interested providers to try them.
  • Discuss data that is available from the registry and uses for data.
  • Provide copy of registry flow sheet for review.
  • At every provider meeting, share something positive, such as a patient case study or tool.
  • Involve providers by asking for their input and ideas.
  • Provide patient comments as appropriate.
  • Be enthusiastic so that others will want to participate.
