
Organization of Health Care:
Make Improving Chronic Care a Part of the Organization’s Vision, Mission, Goals, Performance Improvement and Business Plans
- Link asthma measures and outcomes to the strategic business plan.
- Gain the senior leader’s interest with reports and feedback. Provide senior leader with data about benefits of the initiative, including cost savings and improved patient outcomes.
- Have the team leader or physician champion regularly report progress and results to the Board.
- Show Health Disparities Collaboratives training and promotional video to introduce the senior leader, Board, and staff to improving chronic care.
- Place storyboards in places visible to staff and patients.
- Post senior leader report/graphs so staff can review.
- Discuss with senior leader the direct impact that improved outcomes have on the organization. Provide references of other senior leaders who have undertaken the same effort.
- Help senior leader in determining the “value” of improving chronic illness care (e.g., quality, accreditation/PCER, increased productivity/efficiency, business case, more patients, and important community linkages that can provide needed services and resources).
- Make a standing monthly appointment with the senior leader and team or team leader to discuss report to senior leader.
- Provide report to senior leader two or three days prior to meeting to allow for review time and to remind leader of appointment to discuss.
- Use the Clinical Information System to collect data about cost and outcomes.
- Understand the data! Know what the issues/barriers are, and present them in an objective manner that invites the senior leader to contribute.
- Keep it simple! Provide a simple, straightforward report that everyone can understand. Avoid being too technical or too clinical.
- Quote patients’ own words or reactions whenever possible, or discuss provider satisfaction.
- Provide printed reports and if possible do overheads of graphs.
- Present to Board of Directors a list of barriers to progress—e.g., transportation, equipment needs (spirometers), patient and family understanding of the disease, and resource development—and seek their help in overcoming the barriers.

- Post a sign saying: “For further information, contact xxxxx”
- Assign team member to change posting monthly.
