Delivery System Design:
Assign Roles, Duties, and Tasks for Planned Visits to a Multidisciplinary Care Team
  1. Determine who has responsibility for each step of the planned visit and follow-up.
  2. Obtain senior leader support for training staff in new roles and tasks.
  3. Identify who will do the training.
  4. Identify resources, training material, and tools.
  5. The staff member being cross-trained should shadow or observe the staff member doing the procedure (e.g., inhaler instruction).
  6. Train providers, medical assistants, and nurses in tasks such as peak flow testing, self-management goal setting and follow-up, and periodically check staff competency with all tasks.
  7. Use standing orders for medication refills and immunizations.
  8. Test planned visits using other staff (e.g., nursing staff).

Tips
  • Use staff that volunteer.
  • Do not test too many different roles in too many sites at one time. It is too hard to communicate what is happening.
  • Emphasize to all team members the impact of their role on care.
  • Standing orders do not replace planned visits. Make sure they include monitoring beta-agonist usage and include triggers for seeing a provider.
