The Continuing Care Clinic Handbook is a step-by-step guide to establishing more efficient patient visits.  Visits in which patients see a pharmacist, nurse and doctor, and participate in group education and support all within a periodic half-day visit to your clinic.

This handbook is intended to assist practice teams in using the Continuing Care Clinic model for managing patients with a selected diagnosis or condition, such as diabetes or frail elderly. The handbook is considered a template that can be modified to fit the needs of individual teams.
