This site documents a six-month pilot Collaborative, based on IHI's Breakthrough Series Collaborative model, which ended in April 2004. The Collaborative, funded by The Robert Wood Johnson Foundation and chaired by Doriane Miller of Health Research and Educational Trust (HRET), explored ambitious aims related to patient self-management of chronic conditions. Not all elements of the site are available to the public but the "Tools" section is especially rich.
