A registry is an information system for recording relevant patient care information. A registry allows health care providers to better manage the care for a population of patients. Providers can use registries to record critical elements of the care plan, produce care summaries at the time of a visit, and provide reminders to ensure proactive care. Registries are particularly important in managing the care of patients with chronic disease.

On the last work day of each month, determine the total number of patients included in the registry. If improvement work is focused on a specific chronic disease, determine the total number of patients with that specific chronic disease (for example, patients with diabetes) included in the registry.
