Pilot Collaborative on Self-Management Support
The Pilot Collaborative on Self-Management Support, operated through the Health Research and Educational Trust of Chicago, IL tested the concepts of self-management support in diverse ambulatory delivery settings, resulting in significant improvements in implementation of the chronic care model components that enhance self-management support.

Integrated Outpatient CHF Management Program
The Health Alliance Physician Hospital Organization (HAPHO)-supported team designed and implemented an outpatient program to more effectively manage Congestive Heart Failure (CHF) patients through education, tele-monitoring, and protocol-driven interventions resulting in a reduction of the CHF admission rate by 50 percent and the average length of stay (ALOS) by 2.38 days.
