Vermont Blueprint for Health has committed itself to a strategy that aligns with the Triple Aim framework, and has participated in the Triple Aim initiative since 2007. This session will provide an overview of their strategies and results.

This session will focus on addressing the needs of individuals and families. Topics will include the role of family members as care coordinators, mutual design of care, and empowering patients and families.

Dialog:  System Transformation and Primary Care Redesign

This session will address principles of comprehensive system-level change with an emphasis on redesign of primary care and the implications for workforce skills and development.

Participants will apply principles of individual and family-centered care and primary care redesign to their Triple Aim enterprise.

Triple Aim thinking requires new ways of addressing the measurement of system outcomes, or “big dots.” This session will present options for measuring population health, individual experience, and per capita cost.

Participants will learn how to develop a system for ongoing learning and improvement and also understand how to efficiently customize services based on a segment of the population.

Participants will learn and discuss how to apply the Triple Aim at both a strategic and project level. Some of IHI’s latest work on subpopulations will be discussed.
