Choosing Focus Populations and Segments
During this session, participants select a focus population for the Triple Aim approach, focusing on a population where they have the greatest chance of succeeding.

Viewing the System Through the Lens of an Individual
Participants will develop an illustrative case for their population as a basis for care redesign. After selecting a population, they will describe a case for a person in that population using either a real case that is disguised, a “typical” case, or an extreme but realistic case.

Individual and Family-Centered Care
This session reviews the principles of individual and family-centered care and how they support the Triple Aim. Participants will work on using this design for the case they developed in the morning session.

Primary Care Redesign
Participants will understand the concept of a primary care "platform" and some models for strengthening primary care consistent with the Triple Aim.

Participants will understand the potential role of population health management in health system redesign. Participants will take their case from the prior day and design population health interventions that would help this person.

Participants will be able to apply a robust execution framework to their plans for action. This session emphasizes tying the Triple Aim to current strategies and projects, not adding new projects. The fifth design element, system integration, will be discussed.
