  Seminar Overview

To achieve the Triple Aim, organizations must act as “integrators” focused on achieving high levels of performance across the Triple Aim objectives. In the process, they will address Triple Aim system components that include:

 

  • Individuals and families
    Example: Memory and recognition of individuals anywhere they go in the system

 

  • Redesign of primary care services and structures
    Example: Cooperation and coordination with other specialties and hospitals

 

  • Population health management
    Example: Use of predictive models to deploy resources to high-risk individuals

 

  • Cost control platform
    Example: Yearly initiatives to reduce waste and the associated cost, with all providers involved

 

  • System integration
    Example: Execution, including spread, of strategic initiatives
