Options and Opportunities in an Epidemic of Diabetes

Urbanization, increasing obesity, and lack of exercise are all contributing to a global increase in diabetes.  Accompanying this will be a rise in the complications of heart disease, blindness, strokes, and kidney failure.  This Plenary will demonstrate diabetes to be an exemplar of chronic disease management, which with concerted effort and collaboration between the individual, health care system and society, can improve outcomes for patients.  The Diabetes Audit and Research System (DARTS) in Tayside, Scotland will be demonstrated, with a detailed description of how this is supporting National Diabetes Care.

Information Technology or Practice Transformation -- Which Comes First?  Answer - Both!

Often called one of the pioneers of the electronic medical record, Dr. Leavitt will take on some provocative questions:  Can there be meaningful practice transformation without IT?  Can electronic medical records be successfully implemented without major practice redesign?  And what additional benefits might be achieved when these two disciplines work synergistically?
