To join this Collaborative, please complete the enrollment form and email it to collaboratives@ihi.org.

The yearly fee per entity to participate in a Collaborative is $15,000. A reduced rate of $7,500 per entity per year applies to federally-qualified health centers, physician practices comprising fewer than 20 physicians, and hospitals with an average daily census of less than 50 beds.

Note: The fees to participate in a Collaborative do not include hotel and travel expenses for the in-person meeting that takes place once per year. As this meeting is an integral part of the Collaborative experience, we encourage you to budget these costs into your planning for the program.
