The yearly fee per entity to participate in this Collaborative is $15,000. A reduced rate of $8,500 per entity per year applies to federally-qualified health centers, physician practices comprising fewer than 20 physicians, and hospitals with an average daily census of less than 50 beds.

Note: The fee to participate in the Collaborative does not include hotel and travel expenses for the one in-person meeting that takes place. As this meeting is an integral part of the Collaborative experience, we encourage you to budget these costs into your planning for the program.
