Special Fall Harvest Campaign LIVE!
Join a special Campaign LIVE! on Wednesday, November 7 from 4:00 – 5:00 PM Eastern Time to learn what we learned on the Campaign’s Fall Harvest. IHI staff, members of the Campaign team, and health leaders and experts began our visits to a hospital in every state and the District of Columbia on October 24. At every stop, we’ve come away with new tools, innovative approaches to mobilizing staff on behalf of improvement, and many, many examples of notable progress and results across all 12 interventions in the 5 Million Lives Campaign. This is just the first wave of our efforts to tap the energy and gather up evidence of Campaign participants’ commitment to better patient care. We plan more visits and more ‘harvests’ in the near term.

Tell us about your medication reconciliation success! As part of the Fall Harvest, we’re excited to announce the first IHI Innovation Challenge: Medication Reconciliation. If you’re a Campaign hospital that has been able to successfully implement a sustainable medication reconciliation program, please tell us how you did it. We’d also love to hear from anyone with a fresh idea about how to tackle many of the thorny issues presented by medication reconciliation.
Three winners in two categories, picked by a panel of experts, will be announced at the IHI National Forum in December. Outstanding innovations from all submitting teams will be compiled into a resource that will be presented publicly early next year so that all Campaign hospitals might benefit.
The deadline for submissions is Monday, November 26, 2007 at 9:00 AM Eastern Time, so download the rules and entry form to get started.

Have you gotten to zero? Based on our site visits and progress reports from Campaign hospitals, we believe a significant number of hospitals may have achieved zero central line infections and even zero pressure ulcers for months or years at a time.
To capture this progress, we’ve created two new online surveys, available to all Campaign hospitals, that ask about “getting to zero” central line infections and pressure ulcers. Here are the links:
