DATA SUBMISSION AND MEASUREMENT

In the 5 Million Lives Campaign, we currently plan to measure two things:

  1. The number of acute care inpatient deaths that we would expect to occur in the Campaign period (CY 2007 and 2008) given 2006 levels of care, but did not, because of improvements in care (“Lives Saved”).
  2. The number of incidents of medical harm that we would expect to occur in the Campaign period (CY 2007 and 2008) given 2006 levels of care, but did not, because of improvements in care (“Harm Avoided”)

We will collect “lives saved” data through direct submission of acute care inpatient mortality data from participating hospitals. We plan to collect data on “harm avoided” by conducting retrospective chart review in a representative sample of participating facilities.

The information that immediately follows will give useful insight on measurement strategy in the 5 Million Lives Campaign, which will be augmented in upcoming calls.

FAQ

Overall Mortality Measurement Information

The Campaign will continue to collect and track Acute Care Inpatient Mortality Rate for hospitals enrolled in the 5 Million Lives Campaign.  Intervention level measurement is recommended, but not required.  Measurement information forms for each intervention are available in the How-to Guide documents in the Getting Started Kits and listed individually below.

IHI Global Trigger Tool

Use of the IHI Global Trigger Tool is not a requirement of Campaign participation but is recommended as an easy-to-use method for accurately identifying adverse events (harm) and measuring the rate of adverse events over time.

**June 2007 UPDATE**

In Appendix C on page 35, the calculation for the measure "Percent of admissions with an adverse event" was missing the denominator (Total records reviewed).

The calculation has now been updated in the paper, to read: "Number of records with at least 1 event/Total records reviewed x 100=Percent of admissions with an adverse event."
