IHI’s most intensive front-line improvement work happens in Collaboratives, nine-month programs designed for organizations committed to achieving sustainable change within a specific topic area. 
 

Fees & Enrollment

Enrollment Fee

The fee per team to participate in this Community is $12,000.
 
Note: The fee to participate in the Community does not include hotel and travel expenses for the one in-person meeting during the nine-month program duration. As this meeting is an integral part of the Conversation Ready experience, we encourage you to budget these costs into your planning.
 

Participating sites will receive the following benefits:

  • Guidance on configuring a successful team
  • Tools for assessing underlying infrastructure for Conversation Readiness
  • The opportunity to bring cross-institutional and cross-continuum teams together
  • Opportunities to create a more global and cohesive approach across the organization
  • Peer connection (all teach, all learn) and a community of support
  • Input in defining a new standard for end-of-life care
  • Opportunity to work with media and press around how we deal with end-of-life care
  • Support to build reliable systems and capacity to do this work
  • An approach for integrating efforts already underway: POLST, MOLST, Advance Directives, etc.
  • Education in the new role of palliative care
  • Help with creating leadership buy-in
  • Coaching for your staff to host Conversation Groups at your organization
  • Meeting patients where they are; no wasted effort, higher patient/family satisfaction
 
 

How to Enroll

During the enrollment process you will be asked to include the name and contact information for the roles below. Please have this information available before beginning registration. Organizations can send up to four participants to the face-to-face meeting.
 
Your Key Contact is the “captain” of the organization’s Conversation Ready Health Care Community participation and is often also the Team Leader.
 
Your Team Leader may also be the key contact, or may be another individual who will guide the team in good project management, idea generation, testing, and implementation. This individual will also meet regularly with the organization Sponsor to review progress and results.
 
Your Sponsor is an organizational leader who is responsible for and accountable to the organization for the performance and results of the Conversation Ready Health Care Community team.  This person is not a member of the team, but is responsible for securing the resources that are required and for communicating progress to others in the organization.
