Health Systems Recognized by IHI

​​​​​​​​​​​Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) in partnership with the American Hospital Association​ (AHA) and the Catholic Health Association of the United States (CHA). As of May 2022, there are over 2800​ recognized Age-Friendly Health System participants.

Five pioneer health systems stepped forward to work with IHI to test ideas and learn what it takes to be an Age-Friendly Health System. The 4Ms Framework for Age-Friendly Care that emerged is both evidence-based and able to be put into practice reliably in health care settings.
